Hospital Sisters Health System has your convenience in mind. We are pleased to offer online pre-registration for those patients using one of our hospitals or satellite locations for services.
Our online pre-registration process will save you time upon arrival and allow you to move to your service area promptly. It also ensures we have the most accurate information for your visit. All information submitted on the pre-registration form is completely confidential and secure.
PLEASE READ CAREFULLY BEFORE COMPLETING AND SUBMITTING YOUR ONLINE PRE-REGISTRATION.
The following items are needed in order to complete the online pre-registration form.
- Name of ordering physician.
- Date of scheduled procedure or expected arrival date. (scheduled procedures must be two business days in advance of submitting online pre-registration)
- Personal, employment and contact information.
- Insurance information. Please have your insurance card(s) available.
